The Fair Credit Reporting Act (FCRA) regulates requests by an employer for an applicant's 
consumer report. A consumer report may include such information as an applicant's credit 
payments, driving records, or criminal history. Among other things, the Act requires 
employers to give notice, obtain permission, and disclose certain rights to applicants.
 These forms can help assist you in complying with FCRA and adhere to procedure when 
requesting a consumer report.

Caution: 
Many states have laws which prohibit or limit an employer's use of consumer credit 
reports or criminal records checks and/or prohibit discrimination based on credit or 
criminal history information. Be sure to check the applicable laws in your state and 
consult with an employment law attorney who knows your state laws to ensure full 
compliance.
